Veterinary Care – Any animal, during
transport, may need attention by a veterinarian. If any
such occurrence arises with your pet, we will seek medical
help by a Washington State licensed veterinarian of Fun
Pet Rides L.L.C. choosing and charge your account for the
full costs incurred for any treatment received. These charges
must be paid in full at the time of drop-off of your pet.
Late Charges – Late charges, in
the amount of $20, must be paid prior to any other transportation
reservations. Returned checks will be charged in the amount
of $35.00.
I/we agree to pay promptly, in full, at time of pick-up
of said pet and all charges incurred by Fun Pet Rides L.L.C.
I/we also give permission to any veterinary care center
of Fun Pet Rides L.L.C.’s choice to treat our pet
as deemed necessary for the pet’s health and well-being
and will be solely responsible for payment of any charges
incurred for such care and treatment.
My/our signatures on this form are proof of our acceptance
to all of the terms and conditions and release of liability
clause. Signatures are required before any services will
be rendered by Fun Pet Rides L.L.C. This agreement has no
time limit and is valid and enforceable for any and all
future pet taxi services rendered for our pet by Fun Pet
Rides L.L.C.
I/we certify that ALL owner’s of said pet fully understand
all the terms and conditions and agree to release Fun Pet
Rides L.L.C., it’s owners and employees from all liability
should any illness or injuries, mild or severe, be inflicted
upon or sustained by my/our pet while in the care of Fun
Pet Rides L.L.C.
